Report an Incident

Overview

UFV students and employees can use this service to report an issue or outage relating to a UFV service or system (e.g., email, Banner, etc.).

Information Required

Students and employees can complete the Report an Incident form to report an issue or outage relating to a UFV service or system.

  • Ticket Category
  • Name
  • Department
  • Subject (i.e., short title)
  • Detailed description of the issue
  • Attachment of any screen shots or error messages

Instructions

Click on the "Report an Incident" button and complete all required fields on the form.

Before submitting your incident ticket, be sure to search the Knowledge Base for any support articles related to the problem you are experiencing.

Check the ticket status of your tickets in the Online Portal for updates.
