Report a Cybersecurity Concern

Overview

This service is used to report a cyber security concern or incident that may violate security policies or the Appropriate Use of Information Technology Resources policy (#14). Your report will be forwarded to the Cybersecurity team for investigation and follow-up.

What constitutes a cybersecurity concern?

A cybersecurity concern could be any event that poses a threat to you or someone else, or threatens the confidentiality, integrity, or availability of UFV systems, sites, and data. Examples of a cybersecurity concern include:

  • Compromised UFV account(s) or device(s)
  • You found a bug / vulnerability in a UFV system or website  
  • Theft or loss of a UFV computer, USB stick, data drives, or files
  • Ransomware attempts
  • Online abuse, harrassment, or intimidation
  • You found sensitive data has been stored unsafely, or exposed to parties who should not have access

This list is not exhaustive. We encourage anyone with any cybersecurity concerns to fill out this form with as much detail as you can provide. Your report will be sent to the Cybersecurity team for investigation.

Students and employees can complete the Report a Cybersecurity Concern form to report any concern or incident that may violate cybersecurity policies.

Instructions

Click on the "Report a Cybersecurity Concern" and complete all required fields on the form.

Contact the IT Service Desk, or check the ticket status in the Online Portal for updates.
