Overview
This service is used to request access to MachForm entries and reports. This is typically requested by a MachForm administrator or MachForm data custodian.
MachForm permissions must be approved by the MachForm data custodian (i.e., form owner).
Request this service by clicking "Request MachForm Permissions" in the top right of this page.
Information Required
UFV employees can complete the MachForm Permissions form to request access to MachForm entries and reports. Please provide the following information:
- Department of the person that needs the permissions
- Type of permission needed
- Purpose of access
- URL of the current MachForm
- Name of the data custodian / form owner (approval required)
Instructions
Click "Request MachForm Permissions" in the top right of this page and complete all required fields on the form. An approval will be sent to the data custodian (i.e., form owner) designated on the form. The data custodian needs to approve the request before access will be granted.
