Getting Started

This article walks you through how to fill out an Initial Contact form — your first step toward accessing counselling services.

Note: If this is your first time coming to UFV Counselling, you may also prompted to sign a different form called Informed Consent. You will be prompted to sign an Informed Consent every six months.

Step 1 - Log in to the UFV Student Health & Wellness Portal

Go to ufv.ca/counselling and click on Book Now, or visit studenthealthandwellness.ufv.ca.
Sign in with your UFV student e-mail and password.

Note: If you need help with your UFV log in, visit the UFV IT Service Desk for help.

Confirm your date of birth and click Proceed.

Step 2 - Go to the Survey/Questionnaires Page to Find the Initial Contact Form.

To access the Appointments page, you can:

Click Surveys | Questionnaires in the left-hand menu.
Click Complete a Survey on the Portal home menu.

2. Click on the link to the Initial Contact form.

Note: You can view the date you last submitted the Initial Contact form.

If it has been less than 30 days, there’s no need to complete it again—just check your Messages for next steps from the Intake Counsellors.
If more than 30 days have passed, please complete a new Initial Contact form.

Step 3 - Complete the Initial Contact Form.

Fill out the Initial Contact form. At the bottom of the page, enter your name, today’s date, and click Continue.

Step 4 - Confirm Your Form is Submitted

Check for the Success! message.

Step 5 - Wait For Message From the Intake Team

After we receive your form, the Intake Team will send you a message in the Portal with next steps. To learn how to check your messages, visit Check Your Messages.
You’ll get an email sent to your UFV student email letting you know there’s a message in your portal (and a text, if you signed up). You can update your notification settings by clicking Profile in the portal. Learn how to Update Your Personal Information.
