Overview
This service is used to request a new web form or an edit to an existing web form.

Information Required
UFV employees can complete the Create or Update Web Form to request a new web form or an edit to an existing web form. Please provide the following information when filling out the form:
- Web form approver
- Department
- URL of web form
- Target audience
- Web form new or updated field details
- Attachment of the web form mock up or updates
Instructions
Click on the "Submit Create or Update Web Form Request" button and complete all required fields on the form.
