Overview
An Activity Code Setup or Modification request is necessary when an update or deactivation is needed to an existing activity code or a new activity code is needed.
Information Required
You will need to include the following important information as part of your request:
New Activity Code:
- Description of new activity code
- Effective date
Updated Activity Code:
- Activity code to modify
- Modified description of activity code (if applicable)
- Effective date for change
- Reason for modification
Deactivation of Activity Code:
- Activity code to deactivate
- Effective date for change
- Reason for deactivation
Instructions
Click on the "Submit Activity Code Setup/Modification Request" button and complete all required fields on the form.
