Biosafety Permit Application

Overview

The Biosafety Program at UFV ensures that all faculty, staff, and students know how to safely handle biological agents that can be hazardous to people, animals, and the environment in a way that complies with all regulatory requirements.

The UFV Biosafety Manual provides information to safeguard UFV personnel from any accidental exposure to biohazardous agents.

Principal investigators, faculty, or staff working with any biohazardous material are required to obtain a biosafety permit.  Once submitted, you will be contacted and a local risk assessment will be arranged.

IMPORTANT: This service is only monitored during UFV's regular business hours. 

Information Required

UFV faculty and staff can complete the Biosafety Permit Application to apply for a biosafety permit.  Please provide the following information:

  • Application type and previous permit # (if applicable)
  • Application purpose
  • Principal Investigators / lab instructors
  • Start and end dates
  • Type(s) of biological agents
  • Material handling procedures.
