Overview
The Safety and Security department can create a personal safety plan for anyone on campus including students and employees, contractors, visitors, and guests. Click the "Request a Personal Safety Plan" button to request assistance creating a personal safety plan.
NOTE: This service is only monitored during UFV's regular business hours.
Information Required
Students, employees, contractors, visitors, and members of the public can complete the Request a Personal Safety Plan form to request assistance creating a personal safety plan. Please provide the following information:
- Name
- Student / Employee number (if applicable)
- Department
- Reason for requesting a Personal Safety Plan
- Police file number (if applicable)
- Specific safety measures you would like UFV to take
