New Physical Phone

Overview

Use this service to request a new physical desk phone with a local number (single line), front desk phone (multi-line), or a queue phone. A new local phone with basic phone service is available to eligible faculty and staff. The cost, via a budget transfer, is approximately $532. Approval from the Spending Authority of the budget code provided may be needed.

IMPORTANT:  If you require a softphone, submit this request instead.

The employee submitting this request must be part of the Authorized Account Requestors group.  Authorized Account Requestors include:

  • Departmental Assistant, Administrative Assistant (e.g., Executive Assistant, Program Assistant, etc.), or as authorized by Director, VP, or Dean
  • Department Head
  • Dean
  • VP / AVP
  • Director
  • Manager
  • Employees on the Exceptions List (internal list maintained by IT Services)

Information Required

UFV employees, that are authorized account requestors, can complete the New Physical Phone form to request a new physical desk phone with a local number (single line), front desk (multi-line), or queue phone.  Please ensure you provide the following information:

  • Phone type (local, front desk, queue)
  • Extensions you want to answer (if multi-line)
  • Agent ID (if queue phone)
  • Budget code
  • Spending authority

Instructions
